We are a preferred provider with almost every insurance company.

In order to be sure of your specific coverage; please call the front desk and provide us with your insurance company, your ID or member number, your date of birth, and the subscriber's name and date of birth. We will look up your benefits and provide you with a quote of what your insurance company tells us.

Please be aware that it is our office policy to collect all money up front, at each visit for any deductibles, copays, and co-insurance. Please know that this is an estimate and depending on how your insurance reads and processes the claim, there may be a remainder due or a refund due. We accept Visa, MasterCard, Discover, cash, or check.

We submit bills to insurance companies weekly. However, payment is dependent on your plan guidelines and limitations. We do our best to provide you with the most accurate information, but any plan changes can affect your coverage with us and you will ultimately be responsible for any charges incurred in our office.
